My sisters and my fiance's had to be removed. My sister was in overnight and my fiance was apparently in hospital for a couple of days. Personally I had no hassle with mine.

In my case the CBT came first and then I was given meds (trifluoperazine). The CBT gave me the tools to actually make the first steps and then the meds kind of eased the worst situations.
